Parents and Students Complete Your Activities Registration Online
Please read and follow these steps carefully to be fully registered. You will need to register for one activity at a time.
IMPORTANT: You are only registered after ALL steps have been completed.
Athletic Activities are required to complete all 4 steps below.
Extra-Curricular/Co-Curricular are required to complete Step 2 parts a, b, d and Step 4 if a scholarship is being requested.
Step 1: Athletes must have a current sports qualifying physical exam on file in the Activities Office. Physicals are good for 3 years from date of your last physical. You can get the blank Physical Form here.
Once you have your physical completed, you can turn it in to the office or you can scan it and submit completed Physical Forms online.
Step 2: Read, fill-out and submit the following - all as one step.

Click Family Account Login. Use the username and password you created during your first registration.
Once logged in, click on the ‘Register’ dropdown and register an existing student or add a new student in your account.
Step 3: Montevideo Schools and Chippewa County believe that all students should have the opportunity to participate in activities outside of the school day. Through the Chippewa Champions program, the County has established an activity scholarship fund for Montevideo students. Scholarships may be provided according to the fee schedule below. The family will be responsible for paying the rate listed according to their Meal Benefit status, with the remaining amount being scholarshipped by the Chippewa Champions program. Families requesting a scholarship must first fill out and submit the Meal Benefits Form - Contact the superintendents office at the middle school for further information.
