Parents and Students
Complete Your Activities Registration Online
Please read and follow these steps carefully to be fully registered.
You will need to register for one activity at a time.
IMPORTANT: You are only registered after ALL steps have been completed.
Athletic Activities are required to complete all 4 steps below.
Extra-Curricular/Co-Curricular are required to complete Step 2 parts a, b, d
and Step 4 if a scholarship is being requested.
Step 1: Athletes must have a current sports qualifying physical exam on file in the Activities Office.
Physicals are good for 3 years from date of your last physical.
Blank physical forms can be found here.
Completed physicals can be turned in to the high school office, emailed, or scanned and submitted online (via registration).
Step 2: Read, fill-out and submit the following - all as one step online.
a. Student Information Form
b. MSHSL Eligibility Form
c. MSHSL Health Questionnaire
d. Montevideo General Policies
e. Montevideo Student/Athlete Handbook
Step 3: Payment for athletics/activities
(payments must be made prior to participating)
Step 4: Montevideo Schools and Chippewa County believe that all students
should have the opportunity to participate in activities outside of the school day. Through the Chippewa Champions program,
the County has established an activity scholarship fund for Montevideo students. Scholarships may be provided according
to the fee schedule below. The family will be responsible for paying the rate listed according to their current year
Household Educational Benefits results, with the remaining amount being scholarshipped by the Chippewa Champions program.
Families requesting a scholarship must first complete
and submit the "Household Educational Benefits" form - Contact the superintendent's office
at the middle school for further information

7th/8th graders participating at the 9th-12th grade level will pay the 7th/8th grade fee.

Once you create an online account for a student,
you will need to use that same account each season.
(two different people cannot create different accounts with the same student)

CREATE A NEW REGISTRATION ACCOUNT.
Our registration platform has been upgraded and modified for better functionality.
As with any transition, there may be some growing pains, but we appreciate your patience
and partnership as we move toward a smoother, more streamlined system.
